Chronic Disease Management

National Home Nurse uses a multidisciplinary team approach to deliver your Chronic Disease healthcare needs at home.

We support a range of chronic medical conditions including:

  • Heart disease
  • Diabetes
  • Cancer
  • Stroke
  • Respiratory diseases such as Asthma and COPD
  • Leg ulcers and other chronic wounds
  • Rheumatoid Arthritis and Osteoporosis
  • Multiple Sclerosis
  • Iron Deficiency Anemia

We can help you to manage your condition in your home and improve your quality of life with services such as:

  • Health assessments, including falls risks assessments and pain management
  • Care planning and coordination of care
  • Health and clinical monitoring
  • Assistance with medications
  • Wound care and other nursing services
  • Integrated patient records encompassing services provided by either our Nurses or our Doctors
  • Working alongside your GP or specialist on your care

Hospital in the Home

National Home Nurse is part of 24-7Healthcare, a company that has been providing quality healthcare in the home, in community settings, at aged-care facilities or within a general practice for over 30 years. With our hospital in the home (HITH) programs, our experienced teams provide acute home care services across a range of clinical areas and specialties to facilitate a seamless transition from hospital to home.

HITH services can be delivered wherever the patient lives: at home, in an assisted living facility or in a residential aged facility. Patients who are assessed as suitable for Hospital in the Home can be discharged earlier from hospital, or can avoid hospital altogether and receive the high-level clinical care they need at home.

Services available under the HITH program include:

  • VAC dressings and other Complex wound management
  • Intravenous Therapy- Injections and infusions
  • Rehabilitation, including Physiotherapy and other Allied health services
  • Medication Management
  • Diabetes Management
  • Continence Management
  • IDC/SPC Catheter care

Wound Management

If you have a wound, ulcer, tracheostomy, or stoma and want to continue living in your own home, our skilled nurses can help by visiting you at home to provide the nursing care you need. We deliver wound care services including:

  • Assessments for general health, pain, and wounds
  • Acute wound care such as post-surgery incisions and skin tears
  • Drains, port-a-cath and PICC management
  • Removal of clips and sutures
  • Chronic wound care such as leg ulcers, diabetic ulcers, pressure areas and infected wounds
  • Compression bandaging and stockings
  • Stoma care; assistance with colostomy, ileostomy and urostomy
  • Nasal gastric and PEG care
  • Tracheostomy care
  • Negative pressure wound therapy management (VAC dressing)
  • Working with your specialist, GP, dietitian, stomal therapist, and wound clinics
Our nurses are available seven days a week to ensure that your wounds are treated regularly to optimise healing and you avoid unnecessary stays in hospital.

IV and Infusion Therapy

Patients eligible for HITH care and Pharmaceutical Patient Support programs can receive their treatment, short term or long term injection and infusion services in the home at the same level of care as they would in hospital. Nurses can deliver treatment of variable frequencies to patients, at a time mutually agreed and flexible to lifestyle choices.

IV Therapies for conditions including pulmonary embolisms, deep vein thrombosis and chronic diseases.

IV antibiotics for treatment of cellulitis, community acquired pneumonia, COPD, UTI’s and other infections. Patients that have been discharged without a PICC line in-situ, can be cannulated by our Nurses in the home.

Continence Management

Our dedicated nurses can assist patients with their continence issues, providing clinical advice on how to treat and improve symptoms, discuss the most suitable continence aids, and provide care for catheters.

Services include:

  • Continence assessment
  • Catheter care for indwelling or supra-pubic
  • Continence aids such as pads and catheters
  • Applications for eligible government subsidies for continence aids
  • Education and support
Continence is often a treatable condition with the right support from your doctors and Nurses.

Home Nursing

Our highly skilled and qualified nurses offer a range of home nursing services to allow you to transition back home after a stay at hospital or work with you to manage a short or long term health condition.

  • Chronic Disease Management
    Supporting you to manage your condition at home, in conjunction with your doctors
  • Diabetes Management
    Assistance with care planning, administering medication, wound care, and clinical monitoring
  • Wound Management and Stoma Care
    Changing dressings, managing wounds, and maintenance of stomas
  • Continence Management
    Proving assessment and a personalised care plan for issues with continence
  • Medication Management
    Administering all types of medications, including injections, tablets, and creams

Nursing services can be used intensively for a short time while you recover from an illness or surgery, or long term to support Chronic Disease Management programs. Our services are flexible and can be changed as your needs change.

You can use our nursing services in conjunction with any other services that we offer.
